Coordinated Referral Resources
As our CoC moves forward with developing a system of coordinated referral, the following resources (papers, webinars, slides, etc.) are recommended to become familiar with key concepts and learn how other communities have already begun to structure their own systems:
Why Coordinated Assessment if Critical to Ending Homelessness Locally (HUD)
The National Alliance to End Homelessness (NAEH) has an extensive Coordinated Assessment Toolkit.
In November of 2013, the NAEH presented an hour long webinar that is available for playback or in slide format: Coordinated Assessment: Understanding Assessment Tools
Getting Technical with Coordinated Assessment — Slides from a presentation given by Kimberly Walker of the Corporation of Supportive Housing. (More training resources from the CSH can be found on their website.)
Coordinated Access Overview (slides) From a presentation given by Suzanne Wagner of Housing Innovations at the 2013 NAEH Conference. This gives an overview of Coordinated Intake in the context of HEARTH regulations.
A Brief Overview of the VI-SPDAT – This tool has been implemented by several communities, but others have made informed decisions not to use it, or to use only portions of it. It is one example of how to assess the “severity of service needs” and prioritize chronically homelessness persons for housing placements.
Introduction to the FUSE Model – Another standardized assessment tool, the Frequent Users Service Enhancement model was developed by the Corporation for Supportive Housing, and is in use in a dozen or so communities across the country. CSH is working with several others in the planning stages of adapting this model.
